I consider a large city in civ3 to represent a dominate regional city with a hinterland of smaller cities that have just been abstracted to what is in the game. New York, for instance, could be considered the primary hub for much of the US northeast. ...At least that is how I make sense of the fact that you can have an empire of only 40-50 cities.

I don't think cities of that scale exist underground anywhere on earth.

You're probably thinking of Coober Pedy, which means "white man's burrow" in an Aboriginal dialect. It's an opal mining area. Yes, many buildings are underground there, but I understand that some part of the town is above ground.
