Health Care Help! (Canadians especially)
I need my "health care provider" to fill out some forms regarding vaccinations etc.
Only problem is that the last time I visited a doctor was 6 years ago (I was 17). He was my pediatrician all the way through childhood.
He also had the gall to die 3 years ago and leave me in the lurch. Do I just go to some GP or another, give him the name of my old doctor and whatever documentation I can scrounge up (in Quebec we get little booklets that have our vaccine history in them, IIRC) and hope that the old doctor's records were transferred to provincial or federal authorities?
The pediatrician in question ran his own practice, and he was the only doctor on staff. His wife was his assistant.

Garth is right. See if you can get a physical so that your new GP has some information to work with. If you have any previous conditions that your GP should know, make sure you tell him as well.

Even if the GP you go to doesn't have access to the information you're looking for, he should be able to tell you how to get ahold of your records.

Why do you need a doctor? Waste of money. All you'll ever need are a pair of scissors, a needle, thread and a helluva lot of whisky.

Now...to get an appointment with a new GP in less than a week (since there's a tight deadline on this). He also has to perform a TB test on me...

Give him your old GP's name and town, who ever took
over his practice will have his records (and yours).

If only it was just a medical emergency; I'd just ignore the bleeding till it stopped like I always do.

Clinics often have their own test facilities. If you were in Edmonton I could give you an address.
Try to find a relatively large clinic near you. They should be able to help you out with the testing you need now. Together with your booklet, they should be able to provide the documentation you need. There may be some charge for that. There would be here.
